TOAST分類

こんにちは。

脳梗塞患者さんが、搬送されたときに治療方針決定に役立つのがTOAST分類です。

脳卒中レジデントマニュアルなどにその記載がありますが、いちばん使いやすいのは、個人的にはDecision tree型のタイプだと思います。

医療の現場においては、

Decision tree：

元の複雑なデータの表現をずっと簡単な構造に変換にするのに役に立つ（Wikipediaより引用）

の導入がエラー軽減に有効です。

TOAST Classification of High- and Medium-Risk

Sources of Cardioembolism

High-risk sourcesとしては

Mechanical prosthetic valve

Mitral stenosis with atrial fibrillation

Atrial fibrillation (other than lone atrial fibrillation)

Left atrial/atrial appendage thrombus

Sick sinus syndrome

Recent myocardial infarction (<4 weeks)

Left ventricular thrombus

Dilated cardiomyopathy

Akinetic left ventricular segment

Atrial myxoma

Infective endocarditis

Medium-risk sourcesとしては

Mitral valve prolapse

Mitral annulus calcification

Mitral stenosis without atrial fibrillation

Left atrial turbulence (smoke)

Atrial septal aneurysm

Patent foramen ovale

Atrial flutter

Lone atrial fibrillation

Bioprosthetic cardiac valve

Nonbacterial thrombotic endocarditis

Congestive heart failure

Hypokinetic left ventricular segment

Myocardial infarction (>4 weeks, <6 months)

と定義されます。
